How to Get a Library Card

ONLINE REGISTRATION


The privilege of borrowing books and other materials
is extended by use of a library card. Applicants or parent/legal guardian
library card
(if the applicant is under 18) are asked to show pictorial identification (driver’s license or state id card) indicating their name and current address. In addition, they are asked to show one other form of identification with the same name and current address from the list of pre-approved items. If the picture identification does not contain a current address, then two (2) items containing the same name and current address must be presented. The pre-approved list includes:

Utility bill
Lease
Tax bill
Mortgage payment book
Checkbook
Current vehicle sticker registration
Current vehicle registration identification
Auto insurance card
Voter’s registration card

Persons desiring a library card for themselves or for their children must fill out a registration card in person at the Library. When signing the registration form, the applicant agrees: 1) to be responsible for all materials checked-out on his/her card; 2) to be responsible for any charges (including collection agency fees) that may result from late return, loss or damage of materials borrowed; 3) to comply with the local Library and Metropolitan Library System regulation

In the case of a minor under 18, the parent’s/legal guardian’s signature is also required. This signature indicates: 1) assumption of financial responsibility for materials checked out on the minor’s card; 2) awareness of the fact that there are no restrictions on borrowing any library materials except for AV materials; and 3) acceptance of the responsibility of the minor’s selection of material
